Performance of infectious diseases specialists, hospitalists, and other internal medicine physicians in antimicrobial case-based scenarios: Potential impact of antimicrobial stewardship programs at 16 Veterans’ Affairs medical centers

Objective:
As part of a project to implement antimicrobial dashboards at select facilities, we assessed physician attitudes and knowledge regarding antibiotic prescribing.
Design:An online survey explored attitudes toward antimicrobial use and assessed respondents’ management of four clinical scenarios: cellulitis, community-acquired pneumonia, non–catheter-associated asymptomatic bacteriuria, and catheter-associated asymptomatic bacteriuria.
Setting:This study was conducted across 16 Veterans’ Affairs (VA) medical centers in 2017.
Participants:Physicians working in inpatient settings specializing in infectious diseases (ID), hospital medicine, and non-ID/hospitalist internal medicine.
Methods:Scenario responses were scored by assigning +1 for answers most consistent with guidelines, 0 for less guideline-concordant but acceptable answers and −1 for guideline-discordant answers. Scores were normalized to 100% guideline concordant to 100% guideline discordant across all questions within a scenario, and mean scores were calculated across respondents by specialty. Differences in mean score per scenario were tested using analysis of variance (ANOVA).
Results:Overall, 139 physicians completed the survey (19 ID physicians, 62 hospitalists, and 58 other internists). Attitudes were similar across the 3 groups. We detected a significant difference in cellulitis scenario scores (concordance: ID physicians, 76%; hospitalists, 58%; other internists, 52%; P = .0087). Scores were numerically but not significantly different across groups for community-acquired pneumonia (concordance: ID physicians, 75%; hospitalists, 60%; other internists, 56%; P = .0914), for non–catheter-associated asymptomatic bacteriuria (concordance: ID physicians, 65%; hospitalists, 55%; other internists, 40%; P = .322), and for catheter-associated asymptomatic bacteriuria (concordance: ID physicians, 27% concordant; hospitalists, 8% discordant; other internists 13% discordant; P = .12).
Conclusions:Significant differences in performance regarding management of cellulitis and low overall performance regarding asymptomatic bacteriuria point to these conditions as being potentially high-yield targets for stewardship interventions.

Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.

The Taipan galaxy survey (hereafter simply ‘Taipan’) is a multi-object spectroscopic survey starting in 2017 that will cover 2π steradians over the southern sky (δ ≲ 10°, |b| ≳ 10°), and obtain optical spectra for about two million galaxies out to z < 0.4. Taipan will use the newly refurbished 1.2-m UK Schmidt Telescope at Siding Spring Observatory with the new TAIPAN instrument, which includes an innovative ‘Starbugs’ positioning system capable of rapidly and simultaneously deploying up to 150 spectroscopic fibres (and up to 300 with a proposed upgrade) over the 6° diameter focal plane, and a purpose-built spectrograph operating in the range from 370 to 870 nm with resolving power R ≳ 2000. The main scientific goals of Taipan are (i) to measure the distance scale of the Universe (primarily governed by the local expansion rate, H0) to 1% precision, and the growth rate of structure to 5%; (ii) to make the most extensive map yet constructed of the total mass distribution and motions in the local Universe, using peculiar velocities based on improved Fundamental Plane distances, which will enable sensitive tests of gravitational physics; and (iii) to deliver a legacy sample of low-redshift galaxies as a unique laboratory for studying galaxy evolution as a function of dark matter halo and stellar mass and environment. The final survey, which will be completed within 5 yrs, will consist of a complete magnitude-limited sample (i ⩽ 17) of about 1.2 × 106 galaxies supplemented by an extension to higher redshifts and fainter magnitudes (i ⩽ 18.1) of a luminous red galaxy sample of about 0.8 × 106 galaxies. Observations and data processing will be carried out remotely and in a fully automated way, using a purpose-built automated ‘virtual observer’ software and an automated data reduction pipeline. The Taipan survey is deliberately designed to maximise its legacy value by complementing and enhancing current and planned surveys of the southern sky at wavelengths from the optical to the radio; it will become the primary redshift and optical spectroscopic reference catalogue for the local extragalactic Universe in the southern sky for the coming decade.

Parvicapsula minibicornis is a myxozoan parasite implicated in mortalities of both juvenile and pre-spawning adult salmon in the Pacific Northwest of North America. Disease severity and presentation varies between salmon species and geographical localities. To better characterize population structure of the parasite, we sought genetic markers in the P. minibicornis ribosomal RNA gene. We compared samples from California with the type specimen from British Columbia, identified sequence variations, and then sequenced 197 samples from fish, river water and the parasite's polychaete worm host. Although DNA sequences of the parasite were >98·9% similar, there was enough variation to define 15 genotypes. All genotypes were detected in fish samples, although not in all species. A single genotype only was found in sockeye and pink salmon in the Fraser River Basin, but was not detected in sockeye from the adjacent Columbia River Basin. All coho salmon, irrespective of river basin, were infected with a unique mix of 2 genotypes. These data indicated that the P. minibicornis population exhibited strong signals of structuring by both geography and salmonid host species. Particular genotypes may correlate with disease differences seen in salmon populations in the Pacific Northwest.

Over the past 3 years, we have conducted a survey of 100 square degrees of the southern Galactic plane with the Mopra radiotelescope (HOPS). The survey includes observations of multiple spectral lines in the 12 mm band, with the most important being the water maser transition at 22.2 GHz and the non-metastable inversion transitions of ammonia. We report on initial results from HOPS, including the detection of 540 water masers, about two-thirds of which appear to be new detections. We also find widespread emission in the NH3 (1,1) line, as well as detec tions in the NH3 (2,2), (3,3), (6,6) and (9,9) lines.

Polarimetry at far-infrared wavelengths is a key tool for studying physical processes on size scales ranging from interstellar dust grains to entire galaxies. A multi-wavelength continuum polarimeter at these wavelengths will allow studies of thermal dust polarization in an effort to constrain the grains’ physical properties and test grain alignment theory. High spatial resolution (5–30 arcsec) and sensitive observations will measure the influence of magnetic fields on infrared cirrus clouds, the envelopes and disks of YSOs, outflows from both low- and high-mass star forming regions, and the relative strength of magnetic, gravitational, and turbulent effects in star- and cloud-formation.

Relationships between the X-ray and radio behavior of black hole X-ray binaries during outbursts have established a fundamental coupling between the accretion disks and radio jets in these systems. I begin by reviewing the prevailing paradigm for this disk-jet coupling, also highlighting what we know about similarities and differences with neutron star and white dwarf binaries. Until recently, this paradigm had not been directly tested with dedicated high-angular resolution radio imaging over entire outbursts. Moreover, such high-resolution monitoring campaigns had not previously targetted outbursts in which the compact object was either a neutron star or a white dwarf. To address this issue, we have embarked on the Jet Acceleration and Collimation Probe Of Transient X-Ray Binaries (JACPOT XRB) project, which aims to use high angular resolution observations to compare disk-jet coupling across the stellar mass scale, with the goal of probing the importance of the depth of the gravitational potential well, the stellar surface and the stellar magnetic field, on jet formation. Our team has recently concluded its first monitoring series, including (E)VLA, VLBA, X-ray, optical, and near-infrared observations of entire outbursts of the black hole candidate H 1743-322, the neutron star system Aquila X-1, and the white dwarf system SS Cyg. Here I present preliminary results from this work, largely confirming the current paradigm, but highlighting some intriguing new behavior, and suggesting a possible difference in the jet formation process between neutron star and black hole systems.

We have used the new broadband capabilities of the Mopra telescope to map the distribution of 26 different molecular transitions in an approximately 1 degree square region of the southern Galactic plane (the G333/RCW106 giant molecular cloud complex). The aim is to addresss observationally some of the key questions about the dynamical processes surrounding massive star formation (e.g. massive stellar winds and large-scale galactic flows) and their relative importance in regulating the star formation process. These dynamical processes help drive the turbulent motions, which are ubiquitous in giant molecular clouds (GMCs). The multi-molecular line nature of this survey is what distinguishes it from similar surveys and is crucial for gaining a clear picture of the energetics and dynamics of the gas. Investigating and understanding the chemistry of this region is a necessary part of the project if the molecular line observations are to be interpreted physically.

Polymer/ceramic nanocomposites provide a means of combining the high permittivities (εr) of metal oxide nanoparticles with the solution-processability and high dielectric strength of polymeric hosts. Simple mixing of nanoparticles and polymers generally results in poor quality nanocomposites due to the agglomeration of nanoparticles and poor miscibility of nanoparticles with host materials. We have shown that surface modification of metal oxide nanoparticles with phosphonic acid-based ligands affords robust surface modification and improves the processiblity and the quality of the resulting nanocomposites. We report on the use of phosphonic-acid modified barium titanate (BaTiO3, BT) nanoparticles in dielectric nanocomposites and their applications to high-energy-density capacitors and solution-processable high permittivity gate insulators in organic field-effect transistors (OFETs). Surface modification of BT nanoparticles enabled the formation of high quality nanocomposite thin films with ferroelectric polymer hosts such as poly(vinylidene fluoride-co-hexafluoropropylene), P(VDF-HFP), with large volume fractions (up to 50 vol. %), which are potentially useful materials for electrical energy storage. Similarly, the use of phosphonic acid-modified BT nanoparticles in cross-linked poly(4-vinylphenol) (PVP) allowed to form gate insulators for OFETs. High quality nanocomposite thin films at high nanoparticle volume fractions (up to 37 vol. %) with a large capacitance density (∼50 nF/cm2) and a low leakage current (10−8 A/cm2) were obtained. Pentacene-based p-type OFETs using these nanocomposites showed a large on/off current ratio (Ion/off 104 ∼ 106). We will also present the results from a recent experimental and theoretical study where the BT nanoparticle volume fraction was systematically varied in P(VDF-HFP) host, εr = 11, to find the optimum permittivity and dielectric strength, which provided a guideline for the optimization of the volume fraction for achieving maximum energy density.

How Long Should the Elderly Take Antidepressants?: A Double-Blind Placebo-Controlled Study of Continuation/Prophylaxis Therapy with Dothiepin

Of 219 elderly patients with a major depressive disorder (meeting RDC), 69 recovered sufficiently and consented to enter a two-year double-blind placebo-controlled trial of dothiepin. Survival analysis revealed that dothiepin reduced the relative risk of relapse by two and a half times. Past but not current serious physical illness was also associated with a favourable outcome, whereas a prolonged index depressive illness trebled the relative risk of relapse. In the light of previous research on prognosis it is suggested that elderly persons who recover from a major depressive illness should continue with antidepressant medication for at least two years, if not indefinitely.
